I’d go for the shop approach if theirs an item he wants and he’s in a “safe” place it’s easier for the system to make it but on the fly if theirs a problem it costs an absurd amount so instead of 1000 system points to patch up a wound it’s 100000 while in combat and the more secure he is the less it cost until it goes to base price.
