Gu Ruanci, known in the medical world as the Little Medical Immortal, returns to Dizhou Cheng after ten years of absence. She arrives at the gates of Jing'an Houfu, the estate of her biological father, Marquis Zhou Zhili, to treat the critically ill grandmother. However, she is unrecognized and insulted by Zhou Yuzhi, the second young master, who demands she kneel and apologize for her perceived disrespect. The Zhou matriarch, whose life depends on the Little Medical Immortal's skills, is left without a healer, as Gu Ruanci has explicitly stated she will not treat them. The story concludes with the Zhou family facing social ruin and medical desperation, while Gu Ruanci secures her place in a loving family and begins her campaign of retribution against those who wronged her and her mother.
