Gu Rong is a two-year-old zombie toddler who has survived in the slums of a collapsed civilization under the care of Grandpa Ming. Starving and unable to catch prey due to his small size, he falls asleep under a bridge and wakes up transported into a van. Inside the vehicle are thirteen individuals dressed in hospital gowns and handcuffs, who are actually players in an infinite live-streaming horror game. Gu Rong is not a player but an NPC, a little mental patient bound to System 006. The system informs him that he must complete tasks to earn points, which can be used to fulfill his wish of reuniting with his parents and Grandpa Ming. The game instance is set in Saint Mary's Mental Hospital, where the thirteen players must collect five keys held by the hospital administrators to create a master key and escape, or survive for three days.

Upon arrival, the players are greeted by Chief Morrison, a sadistic police officer, and Sister Ishmael, the head nun of the hospital. Also present is Dr. Lin Xuezhu, a cold and handsome doctor in a white coat, and Jessie, a massive, hairy brute who wields a brush made of his own body hair. The players quickly realize the danger, but their attention is diverted by the presence of the tiny toddler among them. Gu Rong, unaware of the horror context, views the adults as potential sources of food. He attempts to stab several players, including Zhuang Yu and Dr. Lin, with a silver straw, mimicking the action of piercing a milk carton. The players are confused by his behavior, assuming he is just a cute but strange child, while the livestream viewers are delighted by his cuteness, sending gifts to the players who are near him.

The hospital enforces a strict vegetarian diet, claiming that eating meat is a sin that leads to hell. During dinner, a player named 3410 is attacked by octopus-like tentacles that burst from his body after he unknowingly consumes takoyaki, revealing that the food is laced with parasitic entities. The player transforms into an NPC and dies, turning his wristband gray. This event terrifies the remaining players, who realize the lethal stakes of the game. Gu Rong, however, is unbothered and curiously examines a piece of the tentacle before Zhuang Yu discards it. That night, Gu Rong is tasked with scaring a player. He encounters three ghost NPCs—Long-Tongued Ghost, Faceless Ghost, and Skeleton Ghost—who help him execute the task. They hang Gu Rong upside down from the ceiling of player Yang Chunsheng's room, where he performs mid-air aerobics while pretending to be a hanged ghost. Yang Chunsheng faints from the shock, allowing Gu Rong to complete the task and earn points.

The next morning, Gu Rong wakes up cheerful and continues to interact with the players, particularly Zhuang Yu and Shen Hanxiao, who show him kindness by sharing food. The players begin to discuss strategies, noting that Sister Ishmael and Jessie possess keys, while Dr. Lin and Chief Morrison's status is unclear. They also identify the Black Forest in the back mountains as a forbidden area that likely holds clues.
