Wang Tiezhu was once a legendary divine doctor of high status, but after a mysterious incident, he transmigrated into the body of a village fool in Wangjiabao Village. With his intelligence restored, Wang Tiezhu confronts Wang Sipao, who is caught peeping on his sister bathing. He beats the pervert and warns him off, establishing his dominance early on. To pay off the 20,000 yuan debt and save his sister from being sold, he ventures into the dangerous Daniu Mountain. There, he encounters Liu Axiang being harassed by a gang led by Blind Xu. Using his knowledge of the wilderness and a beehive, he rescues her and defeats the thugs. During the escape, they are chased by a massive black bear, which Wang Tiezhu manages to kill using sticky rice dumplings to blind it, securing valuable medicinal parts to sell.

Wang Tiezhu travels to the county town to sell the bear parts but is harassed by corrupt market security officers led by a thug known as Yellow Hair. He swiftly defeats them, drawing the attention of Police Captain Lin Qing. Arrested and interrogated, he uses his medical knowledge to save a suspect who swallowed rat poison, earning the gratitude of the station chief, Zhang Bin, and exposing the corruption of the security officers. Lin Qing, though initially hostile, becomes an unlikely ally after he helps her resolve the situation.

He then visits Linglong Pharmacy, recommended by Lin Qing, where he meets the owner, Li Linglong. After exposing a dishonest employee who tried to underpay him, he sells his goods at fair market value, receiving 70,000 yuan. While there, he treats Li Linglong’s ankle injury and later exposes a medical fraud ring attempting to extort the pharmacy by faking illness. His actions solidify his reputation as a skilled healer and a formidable opponent to criminals. With the debt paid and his family safe, Wang Tiezhu begins to build a new life, leveraging his dual identity as a village hunter and a master physician to navigate the complexities of rural and urban society while keeping his supernatural spatial abilities hidden.
