Park Joo-wan is a high school student who lives in a cramped Seoul apartment with his mother and her other son, his half-brother. His childhood was defined by abuse from his stepfather, who subjected him to physical and sexual violence, and relentless bullying from his half-brother, who harbored a deep-seated hatred for him. The abuse escalated when Joo-wan’s girlfriend committed suicide after being tormented by the half-brother and his gang. In a fit of rage, Joo-wan stabbed his half-brother in the thigh. Rather than seeking justice, his mother had him committed to a rural psychiatric hospital, the Central Mental Hospital, where he was labeled insane and confined for ten years. Inside the facility, he endured neglect and abuse from Dr. Jang Jin-woo, a sadistic doctor who sexually assaulted him in his locked room. Joo-wan survived by dissociating and holding onto the memory of his grandfather’s teachings to never abandon himself.

Dr. Jang Jin-woo, the son of the hospital director, maintains a facade of kindness while secretly abusing his patients. He discovers that Joo-wan has rare RH-negative blood, a valuable commodity in the black market. Dr. Jang colludes with Dr. Kim Je-guk, who is secretly involved in illegal organ trafficking, to sell Joo-wan. They drug him and hand him over to a criminal organization led by Kwon Soo-hyuk, a violent and wealthy CEO who keeps a jaguar as a pet. Kwon Soo-hyuk purchases Joo-wan not for organs, but to keep him as a living blood bag and a captive, citing his distrust of humans. Joo-wan is taken to Kwon’s dark, imposing mansion, where he is forced to eat large quantities of food to regain weight, all while being guarded by the jaguar and Kwon’s subordinates.

Meanwhile, Jang Jin-woo returns to the hospital to find Joo-wan missing. Realizing the severity of the situation and driven by a mix of guilt and obsession, he confronts Dr. Kim Je-guk. Kim reveals that he has been recording Jang’s abusive behavior and uses this leverage to blackmail him, implying that Jang’s own dark secrets could be exposed if he interferes. Jang is trapped between his desire to rescue Joo-wan and the threat of ruin.
